Мета проєкту:

Проект спрямований на управління та координацію роботи мобільної команди з надання консультативних послуг людям з НІЗ, проблемами психічного здоров’я та особам з інвалідністю в Рівненській області; забезпечення медичним обладнанням та виробами медичного призначення.

Стратегічний підхід

НУО (регіональний субконтрактор): організовуватиме та підтримуватиме роботу мобільних медичних команд (ММК), які надаватимуть медичну допомогу вразливим категоріям населення, таким як маломобільні люди, внутрішньо переміщені особи з неінфекційними захворюваннями, а також мешканці відділених та важкодоступних населених пунктів.
До складу ММК входять лікар (сімейної медицини), психолог, медична сестра і водій, які діятимуть у тісній взаємодії з місцевими центрами первинної медико- санітарної допомоги.
В рамках роботи ММК передбачено надання основних послуг первинної медико- санітарної допомоги – огляд лікаря, діагностичні процедури (ЕКГ, тестування крові на вміст глюкози/холестерину та інші дослідження з використанням наданого донором обладнання та витратних матеріалів). ММК здійснюють допомогу пацієнтам в організації доступу до лікарських засобів за програмою «Доступні ліки», у тому числі в рамках співпраці з АТ «Укрпошта», перенаправлення до вузьких спеціалістів тощо.
Робота ММК координуватиметься регіональними координаторами у тісній взаємодії із Департаментами охорони здоров’я відповідних областей та представниками закладів ПМСД регіону.

“If a patient cannot reach a doctor, the doctor must find a way to reach the patient.”
This principle guides the work of the Mobile Medical Teams (MMTs) of the Ukrainian-Swiss project Acting for Health.

Since July, 14 mobile teams have been launched, providing medical services — doctor consultations, examinations, essential medications, and psychological support — to people living in the most remote and hard-to-reach settlements.
Each team includes a family doctor, a nurse, and a psychologist.

In Lviv, Rivne, Khmelnytskyi, and Cherkasy regions, we are testing a new approach: the teams are formed directly on the basis of local Primary Health Care Centers (PHCCs). We provide transportation so that doctors can visit their declared patients. After the project activities are completed, all medical equipment remains with the PHCCs as charitable support.

Our goal is to ensure that every person, even in the most remote village, can access medical and psychological care.
Over the past three years, mobile medical teams have provided assistance to more than 80,000 people — and they remain committed to ensuring that healthcare is accessible to everyone who needs it.

Project Goal

The project aims to manage and coordinate the work of a mobile medical team providing consultative services to people with non-communicable diseases (NCDs), mental health conditions, and persons with disabilities in the Rivne region, as well as to ensure the provision of medical equipment and medical supplies.

Strategic Approach

The NGO (regional subcontractor) will organize and support Mobile Medical Teams (MMTs) that deliver medical assistance to vulnerable population groups, including people with limited mobility, internally displaced persons living with NCDs, and residents of remote and hard-to-reach communities.

Each MMT consists of a family doctor, a psychologist, a nurse, and a driver, working in close cooperation with local Primary Health Care Centers.

As part of their operations, MMTs will provide essential primary health care services such as medical examinations, diagnostic procedures (ECG, blood glucose and cholesterol testing, and other assessments performed using donor-provided equipment and consumables).
The teams will also support patients in accessing medications under the Affordable Medicines state program, including through cooperation with Ukrposhta, and will arrange referrals to specialized care when needed.

The work of MMTs will be coordinated by regional coordinators in close collaboration with the regional Departments of Health and representatives of local PHC facilities.
